It's just an image editor, in my case I use Fireworks.

The air:
First I took the original image of the character, then you pass the eraser for hair. The pen is then used to create the hair, this is done separately, not on image.

Outfit:

From citizen outfit, muscles (chest) and arms are made from assassin outfit. For clothing, the image is copied and clothing is cut separately to modify,then it's about shape and details.
